The Arizona Health Care Association has partnered with Empira to bring evidence-based quality improvement fall prevention and solutions. In this forum participants will meet Mable, a long-term care resident, and follow her through a typical day.  Unfortunately, Mable, like many residents that we care for, experiences a fall.  In order to understand why she fell and to enable you to put in place meaningful interventions we will learn the elements of Root Cause Analysis and explore how our every day practices may contribute to falls in our buildings. We will learn how knowing Mable on a deeper level will help us develop a plan that not only is likely to decrease future falls, but also provide her true person-centered care through meaningful engagement and restorative sleep. Over the course of this forum participants will develop a tangible work plan, individualized to their communities, to help them impact falls in their buildings.

This forum includes an overview of the regulatory requirements both State and Federal for Post-Acute and Long-Term Care facilities and State requirements for Assisted Living Centers. This forum will also cover how surveyors investigate falls and how they identify if a fall is classified as an accident or neglect.
