General Information
Two outstanding Pre-Intensives…
On Tuesday morning, August 26, from 8:30 to 11:45 am, there will be two pre-conference intensives, one focused on assisted living and the other on skilled nursing. Note: a separate registration fee is required to attend.
Registration Fee Includes…
Up to 12.5 CEU hours of educational workshops and keynote sessions, conference materials, exhibit hall, welcome reception, Silent Auction, and all designated meal functions. An additional 3 CEUs are available by attending the pre-conference intensives. Attendance at concurrent sessions is limited to room capacity. An

Cancellation/Refund Policy…
If it is necessary to cancel your conference registration, you must send a written request by fax (602-265-4401) or email (ldoescher@azhca.org) by August 12, 2008. Phone cancellations will not be accepted. Cancellations on or before August 12, 2008 will entitle you to a full refund minus a $50.00 administrative fee. No refunds will be issued after August 12, 2008.

Make your own reservations directly with the Renaissance Glendale Hotel by calling (623) 937-3700. A special rate of $90.00 per night, single or double, plus taxes and applicable fees, has been extended to AHCA. Hotel rates are guaranteed only through FRIDAY, AUGUST 1, 2008, so be sure to make your reservations early.
